ONA Voluntary Long-Term Disability Plan

Could you and your family pay your monthly expenses, if you were unable to earn an income? Long Term Disability coverage is critical to your financial security and to your own independence. Should you become ill, it may be too late to find a solution. Consider your need for income protection now, while you are healthy and actively at work.

The ONA Voluntary Long-Term Disability Plan has been developed specifically to meet the needs of members who do not have employer-sponsored coverage. If you are leaving an employer-sponsored Long Term Disability Plan, you have 30 days to apply for automatic coverage. Additionally, the Plan offers the flexibility of an individual plan, at competitive group rates.

How much insurance should I buy? You need enough coverage to replace your take home income and meet your monthly expenses, in the event that you are unable to work due to illness or injury. The benefit when received is tax-free. The maximum amount you may apply for is 60% of last year’s average gross monthly income. Coverage is available in units of monthly disability income of $250 to a maximum of $2,250 per month, which provides a total of $2,500 including your Base Plan coverage. Benefits will be integrated with other sources of income you receive while disabled.

When would benefits begin and end? If you should become disabled, your benefits begin once you have been totally disabled for 17 weeks or at the end of any sick leave period, whichever is later. This is called the Elimination Period. Totally disabled means you are unable to perform a substantial portion of the duties of your own regular occupation for the first 24 months of disability, immediately following the Elimination Period. After that, you will be considered totally disabled if you cannot work at any occupation for which you are qualified by education, training or experience. Benefits continue for as long as you are totally disabled from a covered illness or injury or until you retire or reach age 65, whichever is earlier.

Do I continue to pay for my Long Term Disability coverage if I become disabled? No. Your premiums will be waived while you are receiving benefits.

What if I need rehabilitation? Depending on your disability, you may need to undergo a rehabilitation program to help you reenter the workforce. In that case, your benefits will be integrated with your rehabilitation income for up to 12 months or to the end of the first 24 months of disability, whichever comes first. (Rehabilitation programs must be approved by Manulife Financial.)

What if I have a partial disability? You may suffer a disability (for which you receive benefits) and recover enough to be able to return to work in a reduced capacity. If that should happen within the first 24 months of your disability, and as a result, you suffer continuous loss of income of greater than 25%, your disability benefits will continue but will be paid in proportion to your loss of income.

Will I be covered for pre-existing conditions? If you have received medical treatment or advice for any condition during the 12 consecutive months immediately prior to being insured, you are not covered for that condition until you have been insured and treatment-free from that condition for 12 consecutive months, or until your coverage has been in effect for 24 months, whichever comes first. New conditions that arise after you are insured by this plan are covered immediately.

How much will it cost? The premium is based on your age as of the effective date of your coverage and the number of units you select. NOTE: The definitions and provisions that apply to the Voluntary Long Term Disability insurance also apply to the Base Plan Long Term Disability coverage.

Life Insurance Plan

Have you considered how you or your family would manage financially, if you or your spouse should die prematurely?

You and/or your spouse each need five times your own annual income, from all sources, to be adequately protected. Is your family adequately protected now?

If not, you and your spouse may want to consider the ONA Life Insurance and Personal Accident Plans. These plans offer convenient coverage, at low rates.

As an added feature, you can continue coverage even if you are no longer an ONA Member.

Some people look at life insurance as a way to leave money to loved ones. But it’s much more than that. It’s a way to protect your loved ones from a financial crisis if you should die prematurely.

How much insurance can I buy? Coverage is available in units of $25,000 (with a minimum purchase of $50,000). You and your spouse can each buy up to a maximum of ten units or $250,000. As a rule of thumb, you and your spouse each need a minimum amount of life insurance from all sources equal to at least five times your annual income.

Plan highlights

  • Waiver of premium — If you or your spouse become disabled before age 60, while insured, you can apply to have your life insurance premiums waived. Upon approval and after six months of disability, coverage will be continued without any further premium payments to age 65 or until you recover, if earlier.
  • Portable coverage — ONA Life Insurance stays with you. If at some time in the future, you are no longer an ONA member, you and your spouse can maintain your coverage provided you continue to pay the premiums.
  • Flexibility — You can choose different coverage amounts for yourself and your spouse. And you can purchase spousal coverage without member coverage.
  • Living benefit feature — This feature can help ease the financial burden of a terminal illness by providing an ”advance” of up to 50% of coverage (to a maximum of $50,000) should you or your spouse suffer such an illness while insured.
  • Termination — Coverage continues to age 65, so long as you continue to pay the premiums.

Extended Health and Optional Hospital

The plan will reimburse you for up to 80 per cent of eligible expenses within your home province. There is a deductible for prescription drug coverage which is equal to your pharmacy’s dispensing fee. No other eligible expenses are subject to a deductible.

What’s covered? Reimbursement of the following eligible expenses is based on the reasonable and customary fees in your home province. All maximums are per person, per calendar year, unless otherwise noted.

  • Prescription drugs — drugs that legally require a prescription and are dispensed by a pharmacist. The annual maximum is $1,200. The lifetime maximum is $30,000. The plan also covers non-prescription drugs required to treat diabetes and Parkinson’s disease. Note: The following are not covered — fertility drugs, treatment for erectile dysfunction, smoking cessation aids, anabolic steroids, or drugs that can be purchased without a physician’s prescription whether or not your physician has prescribed them.
  • Vision care — prescription eyewear (lenses, frames and contact lenses) are covered to a maximum of $200, and one eye exam, every two calendar years (one year for dependent children).
  • Professional counselling services — including stress, anxiety, depression, substance abuse, family/marital concerns, bereavement, work/career problems, and child care and elder care issues (through the ONA Member Assistance Plan).
  • Other professional services — including those of a licensed physiotherapist, chiropractor, osteopath, speech therapist, podiatrist, or registered massage therapist (recommended by a physician) to a maximum of $300 per year per practitioner. In addition, $50 for one x-ray per year per practitioner.
  • Private duty nursing — in-home nursing care by a registered nurse, registered nursing assistant, licensed practical nurse, or certified nursing assistant is covered when authorized by a physician, to a yearly maximum of $10,000 ($5,000 if over age 65).
  • Aids and appliances — the purchase, rental or repair of medically necessary items including: hospital beds, braces, wheelchairs, prosthetic appliances, diabetic supplies, respiratory equipment, Transcutaneous Electric Nerve Stimulator (TENS machines), and orthotics.
  • Ambulance – emergency ground ambulance to and from the nearest medical hospital.
  • Dental care for accidents — treatment required as a result of an accidental blow to the mouth.
  • Hearing aids — purchase or repair of hearing aids to a maximum of $400 every three years.

Optional Hospital can be added to the Extended Health Plan to cover the cost of semi-private hospital room upgrades. Coverage begins on the 1st day of hospitalization, and pays 100% of the cost of a semi-private room for a maximum of 180 consecutive days in a calendar year. The lifetime maximum is $30,000.

Dental Plan

Good oral health is an important element of your overall well-being.The ONA Dental Plan may be just what you and your family need. This plan will continue to protect you throughout your retirement.

What’s covered? Reimbursement of eligible expenses is based on the current dental fee guide for general practitioners in your home province. All maximums stated are per person, per calendar year. The plan covers:

  • Routine care including preventive services, such as exams, cleanings and fillings
  • Endodontic & periodontic services such as treatment for diseases of pulp tissue and gums
  • Major restorative services, for example, crowns, fixed bridges and dentures
  • Orthodontic services for children up to age 19

Coverage

Benefits include: Routine care, Endodontic & periodontic treatment as well as major restorative services (70 per cent of all eligable expenses).

Annual maximums for above services combined: 1st calendar year, $800 2nd calendar year, $1,000 3rd calendar year and thereafter, $1,200 Orthodontia for children (to age 19) is 50 per cent. Lifetime maximum* $2,000 * Orthodontic coverage becomes effective on the policy anniversary date (January 1) after your coverage has been in force for two calendar years. For example, if your coverage was effective May 1, 2002, your orthodontic coverage will be effective January 2, 2004.

Accidental Death and Dismemberment

A serious accident - even if you survive - can have a devastating impact on your finances.The ONA Accidental Death and Dismemberment Plan provides you with one more way to safeguard your family’s future.

How much insurance can I buy? Coverage is available in units of $50,000 (with a minimum purchase of $100,000). You and your spouse can each buy up to five units or $250,000.

How would benefits be paid? If, while insured, you or your spouse should suffer a loss, you would receive full or partial value of the units purchased, as shown in the schedule below: Schedule
